If your child requires any medication during school hours, you must sign a Medication Consent Form.  

Please bring the medicine and the completed form to reception. The medicine will then be kept in a locked cupboard and administered as per the dosage instructions.

Please note-Medical form OA4 is for trips and visits which take place outside the school premises.
